Get in Touch to Start Your Recovery
If you have any questions related to these services, please call me at 810-256-0016 or email me at smithptatdpt@gmail.com. I am happy to discuss any therapy related and service questions with you!
If getting out to the gym is too challenging for you, we can set up an in-home visit (depending on your location) or via telehealth. Please ask! The most important part is to get you the quality care you deserve.
Do you take insurance?
Smith Physical Therapy and Sports Medicine is an in-network provider for many different types of insurance including: Medicare, Medicaid, Blue Cross/Blue Shield PPO Trust, Blue Preferred Plus, Medicare Plus Blue, Aetna, Confinity and ASR. I am always willing to partner with other insurance companies if requested. Please reach out with any requests. If you do not have insurance, cash/check/HAS/FSA can be used to pay for services.
Do I need a physician’s referral?
Michigan is a direct access state which means that you can be seen for physical therapy over a period of 10 visits or 21 days
before needing a referral from their physician for continued care. Most of the time with great personalized care, you feel better before this timeframe expires. If we need follow up care following this timeframe, we can request a prescription from your provider!
What forms of payment are accepted?
Any co-payments are due at the time of service. I currently accept cash, checks as well as HSA and FSA payments.
